Records Requests and Confidentiality Waivers
RECORDS REQUESTS
To obtain records that contain personally identifiable information about a student who is or was enrolled in an educational institution, the Family Education Rights and Privacy Act (FERPA) requires that the party receiving the information agree not to disclose that information to any other party without the prior consent of the personally identified student. Graduate schools, employers, parents, and other individuals do not have access to your records without written permission. Per the Statement of Student Rights and Responsibilities, "Confidentiality of records will be maintained to the extent permitted by law and the University of Michigan Student Rights and Student Records Policy." Since OSCR protects your confidentiality to the extent permitted by law, you must complete and sign a waiver permitting your records to be shared with another person. You must specify who that person is, and what may be shared.
OSCR holds the University's non-academic disciplinary histories for all students and alumni across campus. If you would like to request your record for personal use, graduate/professional school applications, or any other reason, please complete our Records Request and Confidentiality Waiver Form. OSCR offers two different records requests: a summary of your non-academic disciplinary history and a full educational record. As a note, summary requests are the most common and are sufficient for completing requests for study abroad programs, graduate/professional school applications, or other third-party records requests. You should only request a full educational record for personal use as this report may contain information that would not be reflected in a summary of your non-academic disciplinary history. Summary requests are generally fulfilled within 5 business days. Full Educational requests can take up to 45 business days. Your student record will not appear as part of your academic transcript, except in cases resulting in suspension or expulsion.
Please note that OSCR does not maintain academic records. To request your academic record you should reach out to the academic unit in which you are/were enrolled.
CONFIDENTIALITY WAIVERS
For students who wish to request a Confidentiality Waiver allowing OSCR to share information about your involvement in an OSCR process, please complete the form above. This is mostly commonly used when requesting an advisor in our Adaptable Conflict Resolution (ACR), Formal Conflict Resolution (FCR), Informal Conflict Resolution for Alcohol and Other Drugs (ICR4AOD), or Sexual and Gender-Based Misconduct Response programs. Your advisor will also need to complete an Advisor Agreement, which we will process after receipt of a Confidentiality Waiver from you.
Workshops
OSCR offers workshops for student organizations, other U-M departments and units, and academic programs. These educational workshops promote conflict resolution skills for attendees, including conflict styles, communication methods, and active listening techniques. Workshops are by request and we will meet with you to adapt our content to fit your organization/unit/program's needs. Please complete our Workshop Request Form to get started. Once we receive your submission, we will reach out to you to begin planning your workshop. Generally, we meet with workshop requestors through Zoom to identify session content, modality, and determine other logistics for your request. Please submit any request at least two to three weeks in advance of your intended workshop date to allow OSCR to meet with you, identify facilitators, and create session slides.
